Humoral Correlates of Risk for SARS-CoV-2 Breakthrough Infection Following mRNA Vaccination

At World Vaccine Congress Europe 2026, Lenny Moise, PhD (Chief Scientific Officer at SeromYx Systems), presented findings from a study investigating the role of Fc-mediated antibody functions in SARS-CoV-2 breakthrough infections following mRNA vaccination.
While binding and neutralizing antibodies are established correlates of protection against SARS-CoV-2, they do not fully explain why some vaccinated individuals remain susceptible to infection. The presentation explored how additional antibody characteristics, particularly Fc-mediated effector functions, may provide important insights into immune protection and breakthrough infection risk.
Using samples from the SARS-CoV-2 Immunity and Reinfection Evaluation (SIREN) cohort, the study characterized 195 Fc-related antibody features per sample, including antibody isotypes, Fc receptor binding, and functional activities such as phagocytosis, cytotoxicity, and complement deposition. This comprehensive Systems Serology approach enabled the identification of antibody features associated with breakthrough infection at different time points following vaccination.
The findings revealed that specific Fc receptor binding profiles, particularly FcγR3A binding relative to total IgG and FcαR binding relative to IgA1, were associated with infection status. These relationships varied across post-vaccination time points and SARS-CoV-2 variants, highlighting the complexity of functional antibody immunity and the value of looking beyond traditional antibody measurements.
Overall, the presentation reinforced the importance of
understanding not only how much antibody is produced following vaccination, but also how those antibodies function. By incorporating comprehensive Fc-mediated functional profiling, Systems Serology can help identify additional correlates of protection and susceptibility, providing valuable insights to inform next-generation vaccine development.
